FULL FEES

$275.00 for Evaluation, $200.00 for Psychotherapy, $150.00 for Medication

INSURANCE

The practice is currently contracted with the following carriers:

AETNA, UBH/USBH/Pacificare, Healthnet/MHN , BLUE CROSS, BLUE SHIELD

CIGNA is not accepted. New or existing patients with Cigna will have to pay the full fee in the office, and the submit our invoice to be reembursed later by their insurance carrier.

If the practice is contracted with the patient’s insurance panel, then the insurance can be billed. The patient will be responsible for the co-pay as arranged by the insurance company, and the practice will be responsible for submitting the balance of the bill. The patient will be responsible for communicating with the insurance if there are difficulties.

NOTE: If the insurance cannot be billed, the patient will be responsible for the full fee.
